If you would like to make your own appointment on-line click on the link below client portal. Register for a your cliemt portal. When you get into the portal you will see a drop down menu that lists all of our therapists. While making your appointment be sure to click on the name of your therapist.
New Clients
Please download the forms below, You can either print them out or fill them out online. The paperwork in the form of interactive pdf format so it can be filled out online and digitally signed. When you have completed the forms please email them to karen@studiocityclinicalassociates.com. Thank you
